Welcome to Soldiers' / Complex Continuing Care

The Complex Continuing Care (CCC) program at Orillia Soldiers’ Memorial Hospital (OSMH) is for patients who have recovered from the acute phase of illness. Care is continued to help patients achieve goals of rehabilitation, independence, nursing home, or dying with dignity.

 

Our CCC unit has 48 beds, distributed between the fourth floor of Harvie Wing and the Community Tower. Care is provided by our team of geriatrician, nurse practitioner, registered practical nurses, registered nurses, dietitians, occupational therapists and physiotherapists, recreation therapist, clergy, pharmacist, and volunteers, who play a large role in helping patients reach their goals. Families are encouraged to take an active part in patient care.

 

We offer the following programs:

  • Sub Acute Care which helps patients become independent enough to return home with community support services, if needed.
  • Complex Care which provides continued care for patients who need special nursing skills or equipment not available in another setting.
  • Palliative Care which promotes comfort, pain control, spiritual, and emotional support, with a multidisciplinary team approach.
  • Chronic Hemodialysis which supports patients with chronic renal failure who are often from out-of-town and need nursing care or ongoing supervision between treatments.

For patients in chronic care or for those waiting for a nursing home bed, a Chronic Care co-payment fee is mandated by the government, like a nursing home payment. Our Discharge Planner or Business Office Accounts staff will discuss this with you.
