The Geriatric Day Hospital (GDH) at OSMH is an outpatient program designed to maximize independence and quality of life for older adults living in the community.
You and your family will take an active role in your assessment and therapy program. Our treatment sessions take place in a friendly and supportive environment where you are empowered to reach your goals. Each patient is assigned a care coordinator as a contact person.
The GDH is geared to older adults who have the potential to improve in the following areas:

Mobility, physical function and/or falls prevention

Coping with multiple chronic illness or multiple medications

Self-care /Activities of daily living

Living safely in one’s own home

Memory /Cognition
-
Emotional and/or social concerns
-
Communication or swallowing problems

Bowel and bladder function
-
Caregiver stress
-
Navigating the health care and community resource system
All GDH services are covered by OHIP; however transportation and associated costs are the responsibility of the patient/family.
How do I register for the GDH?
Referrals may be initiated by a physician. The client or substitute decision maker must agree/consent to the referral. The geriatrician/geriatric psychiatrist will only consult on referrals signed by a physician.
What are the Referral Criteria?

Service requirement of at least two team disciplines (geriatrician / geriatric psychiatrist, nurse, physiotherapist, occupational therapist, speech language pathologist, dietitian, social worker)

Ability to attend two half-hour sessions twice a week for 6-8weeks (Duration will be determined by the multidisciplinary team)

Treatment goals attainable in an outpatient setting
