The Rehabilitation Day Hospital (RDH) at Orillia Soldiers’ Memorial Hospital is an outpatient program designed for adults requiring ongoing rehabilitation, community reintegration and functional independence enhancement.
Using a team approach, our patients are encouraged to be active participants in their treatment programs. The program goals are to restore, enhance and support the patient’s maximum level of independence with individual and/or group intervention.
Our goal is to provide client/family centered care, compassion, equality, integrity and professionalism to individuals requiring further rehabilitative services due to the following:

Mobility, falls and physical function

Stroke or other neurological diagnosis

Post hospital discharge follow-up

Self-care/Activities of daily living

Communication and swallowing problems
- Memory and cognition

Need for coping ability enhancement
All RDH services are covered by OHIP; however transportation and associated costs are the responsibility of the patient/family.
How do I make a referral?
All referrals must be initiated and signed by a physician. Please see the documents below for more information.
Health Team Members
Our multidisciplinary team is made up of an Occupational Therapist, Registered Nurse, Social Worker, Physiotherapist and a Speech-Language Pathologist.
