(Wednesday, March 6, 2013 - Orillia, ON)  – Do you know a nurse who has made a difference?  If so, consider nominating them for the 2013 OSMH Nurses Alumnae Nightingale Award.

We’re asking residents of Orillia, Oro-Medonte, Severn and Ramara townships to nominate a special nurse – an RN, RPN or Nurse Practitioner – at Orillia Soldiers’ Memorial Hospital (OSMH) for the 2013 Nightingale Award.

Submit your nomination to us, including the following information:

  • First and last name of the nurse
  • What department in the hospital the care with the nurse took place
  • Approximate dates of care provided, or date of association with the nurse
  • Your name and daytime telephone number
  • Your relationship to the nurse (i.e. patient, family member of patient, friend of patient, former student, or colleague. Please note: nurses cannot be nominated by a member of their family).

Then, in 250 to 500 words, provide a personal account explaining why this nurse deserves to be recognized with the Nightingale Award. You may include a brief description of his or her qualities, care of patient, specific examples of the nurse’s efforts and dedication, and the difference he or she has made to your life or the lives of others. (NOTE: Your story must have taken place between January 1, 2012 and December 31, 2012).

Send your completed nomination to c/o Community Relations, Orillia Soldiers’ Memorial Hospital, 170 Colborne Street West, Orillia ON L3V 2Z3, or via e-mail to info@osmh.on.ca by Friday, April 5, 2013. For any inquiries about this award please contact 705-325-2201 ext. 3617.
