Cardiac Rehabilitation

The OSMH Cardiovascular Rehabilitation Program is an education and exercise program designed to support individuals with heart disease after an acute cardiovascular event, such as a heart attack, and after a cardiac procedure or surgery.

Referral Process

Participants must be referred to the Cardiac Rehabilitation Program by their Family Physician or Specialist.

  • Experts agree that virtually all individuals with heart disease can benefit from cardiac rehabilitation.
  • Research has demonstrated that individuals who participate in cardiac rehabilitation can reduce their risk of future heart events by up to 25%


Pulmonary Rehabilitation

Please be advised that the OSMH Pulmonary Rehabilitation Program is currently paused. Referrals directed to this program’s previous fax number (currently the Cardiovascular Rehabilitation fax number) will not be contacted at this time. Referring providers may direct questions to Katie Shum, Coordinator & Exercise Therapist, Cardiovascular Rehabilitation.
