The Regional Kidney Care Program of Simcoe Muskoka is located at OSMH and provides a full range of services for patients with kidney disease. We serve a large area covering west to Meaford, north to Burk’s Falls and east to Brechin.


About The Program

The Regional Kidney Care Program of Simcoe Muskoka provides care for patients across a wide spectrum of kidney disease. Our multi-disciplinary team of nephrologists, nurses, social workers, dietitians, pharmacists and dialysis technologists work with patients and families to ensure excellence in kidney care.

We offer both home dialysis (peritoneal and home hemodialysis) and in-centre hemodialysis. In-centre hemodialysis is provided at OSMH, and our satellite units in Collingwood, Midland and Huntsville. A recently opened home hemodialysis unit on Christian Island helps ensure care closer to home for the Beausoleil Nation.

In addition, we work closely with the transplant programs in Toronto (Toronto General Hospital and St. Michael’s Hospital), Hamilton and London to ensure all patients eligible for kidney transplantation have this option available to them.


Services

General Nephrology and Multi-Care Kidney Clinics

Patients in these clinics are followed for a multitude of kidney-related issues, including glomerulonephritis, pre-dialysis management, hypertension and diabetes.  This clinic offers education to patients and families about kidney disease, how to stay healthy and what to do to delay the progress of their kidney disease. The Multi-care Kidney Clinic also helps patients and families navigate treatment options for kidney failure including dialysis, kidney transplant and comfort-focused care.

Home Dialysis Unit

There are two kinds of dialysis treatments available to patients in their home: peritoneal dialysis and home hemodialysis. Patients and families are taught how to manage every aspect of their home dialysis treatment by experienced team members, and nursing assistance in the home is provided if needed. The Home Dialysis Unit offers close follow-up for all patients to ensure that they are able to manage their treatment safely at home.

Hemodialysis Unit

The program offers in-centre hemodialysis at OSMH and at 3 satellite hemodialysis units across Simcoe Muskoka: Collingwood General and Marine Hospital, Muskoka Algonquin Health Centre in Huntsville, and Georgian Bay General Hospital in Midland.

Transient Dialysis

The Simcoe Muskoka Regional Kidney Program provides in-centre hemodialysis treatments for patients visiting our region, at OSMH or one of our satellite hemodialysis units.

Referral

Our four nephrologists run their clinics from a shared space at OSMH and utilize a centralized referral form.

Referral for nephrology consultation must be sent by a physician or nurse practitioner.

The centralized referral form must be completed and faxed to our office.  Referrals will be triaged based on urgency by our nephrologists. Please note, incomplete referrals will be returned to the referring physician/NP.
