Quality Improvement Plan

Continuous quality improvement – finding ways to do things better – is a commitment that OSMH staff, doctors and leadership make to ensure that our patients have the best possible experience and receive the highest quality care when they come to our hospital.


The Quality Improvement Plan (QIP) is one of the tools that we use to track our performance in areas of patient safety and satisfaction. With this plan, we are able to clearly see our targeted areas for improvement and chart our progress.

Each year, as part of the Excellent Care for All Act (ECFAA) we publicly post our Quality Improvement Plan to support our strategic goal of being a hospital known for patient and staff safety, and to meet community expectations regarding transparency and accountability.

OSMH’s 2024-25 Quality Improvement Plan

  1. Narrative: A brief overview of our strategic goals and areas of focus for the Quality Improvement Plan.
  2. Work Plan: A detailed worksheet that describes our improvement plans, initiatives and targets.
  3. QIP Executive Summary Report 2024 – 2025: A report outlining our performance and progress we’ve made to date on the QIP from the previous year.
  4. Collaboration Report:  A report detailing our collaborations with external partners.
  5. Progress Report:  A report providing details on our performance and lessons learned on each of our indicators from both last year and this year.
