Rehab

The Rehabilitation Day Hospital (RDH) at  Soldiers’ Memorial Hospital is an outpatient program designed for adults requiring ongoing rehabilitation, community re-integration and functional independence enhancement.

Overview

A multidisciplinary team of healthcare professionals work together to maximize our patients’ abilities. A wide variety of services are offered to people needing occupational therapy, physiotherapy and speech language pathology on an inpatient and outpatient basis and require a physician referral. There are fees for some treatment materials, such as splints.

Our therapy Covers:

  • Hand therapy and plastic supports
  • Neurology
  • Cardio respiratory therapy
  • Orthopaedic (including amputee rehabilitation)
  • Rehab Day Hospital (RDH)

Prior to a patient’s discharge, our team will provide our patient and their family with information on community resources and home care support services.

Rehab Day Hospital

Our goal is to provide client/family centered care, compassion, equality, integrity and professionalism to individuals requiring further rehabilitative services due to the following:

  • Mobility and physician function
  • Stroke or other neurological diagnosis
  • Post hospital discharge follow up
  • Self-care / activities of daily living
  • Communication and swallowing problems
  • Memory and cognition

All RDH services are covered by OHIP; however transportation and associated costs are the responsibility of the patient / family.

How do I make a referral?

All referrals must be initiated and signed by a physician. Please see the documents below for more information.
