Quality Improvement Plan
At Orillia Soldiers’ Memorial Hospital (OSMH), continuous quality improvement—finding better ways to do what we do every day—is a shared commitment of our team members, physicians, and leaders. It is a foundational element of our Strategic Plan and guides how we deliver care, support our teams, and partner with our community.
The Quality Improvement Plan (QIP) is one of the key tools we use to turn this commitment into action. It helps us monitor performance, focus our improvement efforts, and understand how well we are advancing priorities related to quality, safety, experience, equity, and system effectiveness.
Each year, in accordance with the Excellent Care for All Act (ECFAA), OSMH publicly posts its Quality Improvement Plan. This supports our strategic goals and reinforces our commitment to transparency, accountability, and continuous learning—so that patients, families, caregivers, team members, and our community can see how we are working to improve care and experiences across our hospital.

- Narrative: A brief overview of our strategic goals and areas of focus for the Quality Improvement Plan. 
- Work Plan: A detailed worksheet that describes our improvement plans, initiatives and targets.
- Progress Report: A report providing details on our performance and lessons learned on each of our indicators from both last year and this year.
