Outpatient Rehabilitation Services Referral Form
The Rehab Day Hospital (RDH) program at OSMH is committed to providing the highest quality rehabilitative services. We endeavour to see all patients on a timely basis, however, it’s important to understand that wait times can vary significantly as patients are seen based on their condition and level of need. Because referrals and patient needs can change, we are unable to estimate when someone will be able to start the program at this time. Thank you for your patience and understanding.
